In the 74th Hunger Games, the female tribute died the first day in the bloodbath at the Cornucopia. The male tribute was placed 10th and died on day 8. He was probably killed by the Careers but could have died of other causes.
The careers had a large pile of suplies from the cornocopia. The boy from district three was left guard it. Katniss blew up the pile. When Cato returned he was so angry at the boy for not guarding the supplies he put his arm around the boys shoulders and jerked his head to the side instanly braking his neck and killing the boy from district 3.
Cato dies while attempting to kill Peeta. He's holding him, and saying that if Katniss shoots him, Peeta will go down with him. She shoots him in the hand, and he lets go of Peeta. He then falls off the roof of the Cornucopia, and is eaten, (or more accurately, torn to shreds), by the mutated dogs.
He dies because in the bloodbath he got stabbed in the leg and he makes it far but his bad leg overcomes him and he losses to much blood
