The Tardis materialized in the middle of a gang shoot-out. He got shot and was taken to a hospital where treatment to save his life actually finished him off due to his non-human Biology. Due to the drugs he'd been administered with before death, his regeneration was delayed, but in the hospital morgue he regenerated into the 8th Doctor.
The Fifth Doctor 'died' from something called spectrox toxemia, caused by accidentally touching spectrox (a toxic substance produced by bats) while wandering around in a cave. The Doctor went into a deeper cave to find some of the antidote but was only able to get enough for one person, and chose to give to his companion Peri, who had also touched the spectrox

There is no episode with the eighth Doctor regenerating, he was only in the 1996 movie, and when the show returned in 2005 it was with the ninth Doctor.
a timelord has 13 lifecycles,but can regenerate more..because in the dr who movie with Paul mcgann..the master had already used his 13th cycle only to take another body,by possesion,then he fell into the eye of the tardis making us all think he died..But then the master returned in the episode eutopia with david tennant having forgotten who he was until he was shot and he regenerated into saxon only to be shot again and die..to be reserected in the end of time,only to return with the timelords once again being trapped in the timelock..So i say the doctor has 12 bodies but as with the master,where there is a will there is a way,and knowing the doctor he will find a way....FANTASTIC
As of February 2015, no.The show is alive and well. in fact the Doctor has only just restarted his regeneration cycles after being granted a "regeneration extension". usually Time Lords are only allowed to regenerate a certain number of times, but Doctor Who is currently an exception to that rule.
I'm not sure but i should think that they should be some sort of Regeneration somtime in the future maybe in 4 years time maybe. I don't know. But kind of hope so. Sounds imposible.

Because it was time for him to go he didn't want to be the doctor anymore and that's why they regenerate so that they can go from actor to actor without it being obvious
