i don't think she dies in the show but during the pain arc she does go into a coma and a old man named danzo becomes hokage and then he dies by sasuke and kakashi almost became the hokage but right when he was going to be the hokage tsunade woke out of her coma.sasuke killed him because after learning the truth of itachi he wanted to kill itachi's enemys which danzo is one of them.sorry for the whole story but no she does not die.
