1. Vic Morrow, Renee Chen, and My-ca Dinh Le were killed during the making of "Twilight Zone: The Movie" (1983). While filming a Vietnam battle sequence involving a helicopter, Morrow was carrying the two children. An SFX explosion sent the helicopter down on top of them, killing all three instantly. Renee Chen and My-ca Dinh Le were not officially part of the cast, and their parents were paid in cash. The director, associate producer, production manager, special effects coordinator, and pilot were charged with involuntary manslaughter. They were found not guilty. 2. Brandon Lee was killed during the making of "The Crow" (1994). A blank from a prop gun shattered his spine.

Usually nobody dies in the making of any movie, Horror or not. Unless there is some freak accident (for example, Brandon Lee was accidentally shot to death during the making of The Crow), or unless there is a murder on the set, no one dies during the filming of a movie.
No movie has ever depicted an actual death on screen. A human death, anyway (real animals were murdered in the making of Cannibal Holocaust...which is why I cannot bring myself to watch that film).
Every horror movie I seen somebody dies but maybe in other horror movies nobody dies.
