No, but they are related. Harry's father, James's great-grandfather was Ignotus Peverell who's niece was Merope Gaunt. Merope Gaunt is Tom Riddle's/Voldemort's mother. So Voldemort is a distant cousin of Harry's dad.

Yes, Tom Riddle is known as Lord Voldemort in the Harry Potter books. Tom Marvolo Riddle is his birth name. During his time at Hogwarts, he sought out his family history and discovered that his father was a muggle who had left his pregnant mother which made him despise muggles, he created his own name which to him severed the connection with his muggle self. He was known by Voldemort by a few close friends while at Hogwarts, but later assumed the name and title completely during his take over of the Wizarding and Muggle worlds.
