It was determined by the National Transportation and Safety Board that the bridge has had structural design flaws from the beginning. These flaws, though not sufficient to cause a collapse during regular use, were sufficient to cause a collapse during the bridge's repair when all of the construction equipment and gravel for concrete making was on it in addition to the load of rush hour traffic.
These were the errors noted in their report:

Mothman is a legendary moth-like creature, who is said to have originated in the West Virginia area. There is a conspiracy theory that the Mothman was seen close to the I-35W bridge collapse in Minneapolis in 2008. The creature was allegedly observed earlier in the month prior to the collapse in Stewartville, Minnesota.
