In a manual transmission between the gears. If one fails it will make it very difficult or impossible to shift in to the lower gear, i.e. if the 2nd gear synchronizer goes out you cant shift from 3rd to 2nd gear
Could be a few different things. Bad shift forks would be the most likely culprit if it were a manual transmission. For an auto, probably either a faulty transmission ECM or worn synchronizer gear.

It depends on the type of transmission. If it is a manual transmission, the first thing to check would be the linkage. If the linkage is not the problem, then it could be the bushings or the synchronizer. If it is an automatic transmission, then it could be a problem with the bands, though you could have debris in the valve body or a servo problem. In some cases, linkage bearings could be an issue.
