How do you find out why your Ford E-350 will not shut off when you turn the key off or when you pull the battery cable?

Pulling the negative battery cable isn't going to do it because the engine isn't grounded through the battery - it has its own ground.
I'd start by inspection the ignition switch itself, as well as the accompanying wire harness for any shorts... any breaks, fraying in the wire, peeling of the insulation, etc.
