The Medusa Cascade was a rift in time & space that was used by The Daleks to store planets in that they had stolen. They stole a total of 27 planets, including Raxiccoricofallipatorius (home of The Slitheen) and it's twin planet Clom (home of The Abzorbaloff), and were going to use them to make a 'Reality Bomb' which would destroy every Universe and every life form except those in the immediate vicinity of the bomb, making The Daleks the only living beings in existence.
The Cascade could not be found, since time inside it was out by a single second. Nonetheless, The Doctor somehow located it and sealed the rift, thus restoring the planets to their rightful places and thwarting The Dalek's plot.
