A decendent of the guy who created the puzzle box, Phillip Le Merchant, used an old design created by Le Merchant using lights and mirrors. He tricked Pinhead onto a space station (in the future) and switched on the 'lights'. This somehow had the effect of destroying the portal his bloodline had created and Pinhead and the other cenobites were destroyed along with it.

He makes a cameo appearance in the red baron story where he is an army officer pensioned off when he drives nails into his head. I think the idea is, he feels guilt at surviving when so many of his compatriots died, so he is trying to make himself suffer. The origins of his maddness.
