The most common way is from excessive heat like a bad thermostat or from low or no coolant. When a vehicle gets several hundred thousand miles on them age can also be a factor.

if you blow a head gasket from overheating you should have the head checked to make sure it isn't warped. especially if it's an aluminum head like most newer vehicles have.
