The requirement varies from state to state, and is sometimes based on the application or special needs of the equipment or services provided.
You need to check with your local health inspector. Another useful resource is EcoLabs. They service our restaurant and their staff is amazingly well versed in the local requirements. Either one is going to be able to tell you exactly how to test the concentrations or how to mix the solution as you need it.
For us its 100ppm-400ppm in the dishwasher and 50-150 for table clothes. We use 1/2 cup of bleach per gallon of water for out table clothes. They also need to be kept in the appropriate red buckets at ALL times, no exceptions.

Mixing things in 'parts' is using a ratio when mixing different things together. It depends upon what you are mixing with it. If you are mixing bleach to water at a ratio of 1 part bleach to 3 parts water, you would add a cup of bleach and three cups of water to equal one quart.
The company says your can use a 10% aka 1:9 ratio of bleach to water safely.

The ratio of the mix is 4:5.... therefore you would need 12 parts bleach to 15 parts water.
If you want to fade your pants lightly you need very little bleach solution. If using 50 percent bleach you need about a tablespoon of bleach per 2 gallons of water.
Safe for what? Generally 10% bleach or a 1:10 ratio to whatever the amount of water. For a percentage example you would add 3% bleach to a 32 ounce spray bottle, you would add 6% bleach to 64 oz (gallon) of water, etc., for a ratio example you would use a mixture of 1 cup of bleach to 9 cups of water, etc. For human consumption 1 Tablespoon for gallon will purify the water and make it safe to drink.
Im in construction in the southern U.S. mainly Texas and we always have mold problems on fire walls and sheetrock. These ratio's have worked wonders for me and saved my companies thousands of dollars im sure. You can do a 50/50 ratio of bleach and water or for even better results add a little TSP. If you have a big job to do get a 5 gallon bucket fill with 2 gallons water 2 gallons bleach and 2 cups TSP. Furthermore for smaller jobs use 3 cups water 3 cups bleach and 2/3 cup TSP. It's a 1/32 ratio (2oz of bleach to 64oz of water), so it's 1/32 of 4, which would be about .125oz of bleach, or a little less than a teaspoon.
