How do you handle a rude restaurant manager when you're the customer?

Walk out without paying, while loudly recommending that the other patrons do the same.

It depends what constitutes "rude" to you. Many customers are asked to leave for inappropriate behaviour (loud talking, inappropriate dress, not turning off cellphones, insulting the staff etc.) Walking out without paying is always an option - bear in mind that this constiutes theft and the manager may have you arrested.
If he restaurant is a chain, or even if it's not, they may initiate a legal proceedings that will cost you big bucks for a lawyer (which they can afford) and fines and potentially imprisonment if you are found guilty. With a record you may find finding a job, getting a passport and getting a loan or mortgage much more difficult.
A more reasoned course of action would be to ask to speak to the owner or manager's supervisor. If this does not work, pay the bill and leave.
If you think that denigrating the restaurant on social media is an option - this may constitute slander or libel which again may result in court action.
