Do women have to remove their hat for the playing of the national anthem?

The protocol is for everyone to remove their hat when their national anthem is played, or when you Another Country's anthem is played. Some exceptions apply - for example, a Sikh's turban or a Muslim's head scarf will likely be left on.
For men, a hat should be removed when you meet a lady and then left off for the rest of your life - nothing looks more ridiculous than a hat.

§ 301. National anthemHow Current is This?(a) Designation.- The composition consisting of the words and music known as the Star-Spangled Banner is the national anthem.
(b) Conduct During Playing.- During a rendition of the national anthem-

(A) individuals in uniform should give the military salute at the first note of the anthem and maintain that position until the last note;
(B) members of the Armed Forces and veterans who are present but not in uniform may render the military salute in the manner provided for individuals in uniform; and
(C) all other persons present should face the flag and stand at attention with their right hand over the heart, and men not in uniform, if applicable, should remove their headdress with their right hand and hold it at the left shoulder, the hand being over the heart; and
(2) when the flag is not displayed, all present should face toward the music and act in the same manner they would if the flag were displayed.

However, in large crowds like sporting events, you'd certainly hear people softly joining in. So, many people sort of do a mixture of mouthing the words or softly singing. (Many people think just standing in a respectful manner with arms at your sides is okay.) And, of course, people still get confused about hats-- should you remove your hat/cap or not? (Yes.)
