The protocol is for everyone to remove their hat when their national anthem is played, or when you Another Country's anthem is played. Some exceptions apply - for example, a Sikh's turban or a Muslim's head scarf will likely be left on.
For men, a hat should be removed when you meet a lady and then left off for the rest of your life - nothing looks more ridiculous than a hat.

(b) Conduct During Playing.- During a rendition of the national anthem-
(1) when the flag is displayed-
(A) individuals in uniform should give the military salute at the first note of the anthem and maintain that position until the last note;
(B) members of the Armed Forces and veterans who are present but not in uniform may render the military salute in the manner provided for individuals in uniform; and
(C) all other persons present should face the flag and stand at attention with their right hand over the heart, and men not in uniform, if applicable, should remove their headdress with their right hand and hold it at the left shoulder, the hand being over the heart; and
(2) when the flag is not displayed, all present should face toward the music and act in the same manner they would if the flag were displayed.

So the short answer is that No women do not need to remove their hats.

In formal etiquette, a soloist in any situation should be able to put on the performance without having a chorus join from the audience. However, in large crowds like sporting events, you'd certainly hear people softly joining in. Partly, it's because the rules ARE confusing: Normally, not singing the anthem is a sign of disrespect, so should you sing the words along with the soloist or just listen? So, many people sort of do a mixture of mouthing the words or softly singing.
