two ways: 1. climb a ladder to remove and replace, 2. there is a tool you can get (Home Depot has them) that has a claw-like device on one end which is attached to an extention pole. A trigger device attached to the pole controls the claw which then grabs onto the bulb while you turn it to remove and replace.
You must push it back into the cavity behind and then remove the wiring and if you wish, seal the hole.

Your blind spot is caused by the spot at which the optical nerves meet at the back of your eyeball. Since there are no Optical sensors at that spot then the area does not respond to light.
