OR, it could apply to the childbirth process. Basically how much the cervical opening has pulled back or thinned. But this is also tied to how much the cervix has dilated, or opened up to allow the child to be born. Fully dilated is 10 cm.

During delivery , the cervix opens ( dilates) and thins (effaces) for a vaginal delivery. For a standard routine delivery you must be 100% effaced and 10 cm dilated. so recording of 4cm dilated and 70% effaced indicated you are entering the final stages of preparing the baby for delivery.
