No desiccation indicates some kind kind of break down in the disk itself. Herniating a disc is to have it moved out of position (pinched or something like that) which can in some cases be fixed.

No. they are different conditions. In disc desiccation you have atrophy of the disc due to degenerative changes. In herniated disc, you have protrusion of the nucleus pulposus in the vertebral canal.
