It depends on the person...some people can smoke all their lives and die normally. while other unlucky people get cancer and die a painful death.

None, unless there was so much smoke that they couldn't get enough oxygen. Other than that, many have died of lung/throat cancer, among other types, as well as complications from weakened immune systems (especially lungs/pneumonia) or from Heart disease. Some people even die from their house burning down. Cigarettes don't kill, cancer does.
