Teaspoons are used as a measure of volume, whereas mg (milligram) is a unit of weight. There is, therefore, no direct conversion between them. It is possible to convert for a given substance, for which the density is known.

In the US, a regulation teaspoon is exactly 5ml in volume, but actual teaspoons can range anywhere from 2.5 to 6ml.

If we assume you are talking about water (for example) and standard teaspoons, then we can calculate the weight of one teaspoon of water to be 5g (since 1ml of water weighs 1g). There are 1000mg in one gram, so there are 0.0025 teaspoons of water in 12.5mg.

As you can see, this is somewhat confusing and so volumetric measurements should be avoided for solid substances and weight measurements should be avoided for liquids.

The answer depends on the substance you're measuring. A milligram measures mass, while a teaspoon measures volume. Equal masses of different materials will have different volumes, so there is no general conversion.

Even different forms of the same substance and different ways of filling the measure matter. Since a teaspoon measures volume, the more air mixed in with the substance, the less substance will fit in the teaspoon, and the smaller its mass will be. A teaspoon of kosher salt, which has large grains that create big air-filled gaps when crammed together, has a lower mass than a teaspoon of table salt, that has smaller grains that fit together more easily. Same substance, same volume, different structure, different mass. If you measure a cup of flour by scooping the measuring cup in and leveling it off, you mix less air into the flour than if you spoon the flour in. That extra air allows less flour in the cup, so the cup of spooned flour will have a lower mass than the cup of scooped flour.
