He escaped by turning into a dog (he can do that beause he is an animagus), slipping through the bars and swimming to sore. The densities are blind so couldn't see him and they only sense humans not animals.This was the first time.

Bellatrix broke out of Azkaban because the Dementors weren't being controlled by the ministry anymore. Voldemort was controlling them. So all the Death Eaters in Azkaban at that time could get out easily, and anyone else in there with them.
