No she did not. But she did lead a clone army as a general during the Siege of Mandalore at the end of the Clone Wars. Ahsoka's death is not yet cannon in the Star Wars universe. In my opinion, it probably happened during Order 66 when anaking was killing the Jedi at the temple. Now, Ahsoka might have not been at the temple. She could've been at a clone war destination and killed by clones. But Ahsoka's fate has not been confirmed just yet. So we'll have to find out.

No. At the end of Season 5, she left the Jedi Order after her name was cleared when she was accused of crimes she didn't commit. The Jedi Order expelled her and turned her over to the Senate. But when they asked her to come back, she refused and left. So that has revealed that her leaving the Jedi has spared her life from Order 66. But what happens to her afterwards is still yet to be revealed
First of all, Ahsoka and Rex's fate are mysteries to be revealed. And if Ahsoka was around during Order 66 and Rex found her, she would be a traitor to him and every clone officer literally open-fired on their Jedi generals.

Twice. In Season 4 Episode 15, Obi-Wan faked his death in order to go undercover for a stealth-like mission, but Ahsoka was able to hold back her tears. In the end of Season 5, when she leaves the Jedi Order, a tear falls down her face as she walks away

How Ahsoka Tano died is still unconfirmed. At the end of the Clone Wars series, she left the Jedi Order shortly before the events in "Revenge of the Sith." Whether or not her decision to leave labeled her a survivor of Order 66 is still to be revealed
