What happens with an erection lasting more than 4 hours?

In some cases yes and in most cases no it is very common depending on the type of medication used, however if you didn't use any medication then is is uncommon and you may want to see your doctor as this could be a sign of a Cardio Vascular Disorder.

Not if you are on Viagra or something similar but if you have it normally you should since it's pressure on the blood vessels in your penis and if it doesn't come out eventually it starts to die. The tissue can't take it anymore. There's a condition some men have and it comes back.

There is a donut-shaped muscle called the penile sphincter at the base of the penis. Normally, blood flows into the penis through arteries deep inside, and out of the penis through veins near the surface. When the penile sphincter tightens, it slows down the flow of blood through the veins, so the penis swells with blood and gets hard.

The problem is, the capillaries (the smallest blood vessels connecting arteries and veins) can't take too much pressure. If the erection lasts too long, a capillary (or several) may burst. This can permanently damage the penis.
