Why does Frodo leave at the end?

He had been through too much during the journey to destroy the Ring and decided that he wanted to leave the mortal lands. He probably had a form of PTSD from everything that had happened to him, and the Undying Lands would allow him to free himself from it.

Frodo decided to leave because he thought the journey was his quest alone, and he did not want to bring harm to his friends. Also, after Boromir tried to take the One Ring from Frodo, he immediately left and Sam always goes where Frodo does.

Frodo leaves Middle-earth because he is emotionally, physically, and mentally wounded in many ways. He becomes depressed and a trauma Survivor, but he always tried to hide it when near his best friend Samwise Gamgee and only reveals his torment when Gandalf asked specifically. His first wound was when the Witch-King stabbed him at Weathertop. Whenever the ringwraiths got near, the pain of his experience returned. It also returns on the anniversary of the fight on Weathertop. Also, the burden of the ring constantly hurt him. It drained his energy and made him see the most horrible images imaginable. And also regret follows him. When about to cast the Ring of Power in Mt. Doom, he lost the will to throw it in because of his "wounds" and he claims it as his own. He feels he failed his quest and his friends. And when he returns home to find the Shire under Saruman's control, he feels he has failed his quest because the only reason he went was to save the Shire and the Hobbits. Finally, he feels the lack of honor. No one even cared about him except the Fellowship and Bilbo. His parents died when he was 12 and at age 21 he went to live with his 99 year old cousin who shared the same Birthday (Sept. 22nd) who was very dear to him. Bilbo left him on Frodo's 33 birthday, his coming of age (which was another reason he was depressed). Frodo suffered so much more than I can explain. A lot of people believe that he left Middle-earth not only because his never healing wounds will begin to heal, but, also, to pay the debt of his claim to the ring.

Frodo suffered from his wound received a Weathertop. It continued to pain him after all those years. He also never fully recovered from the affects of the One Ring. Going to the Undying Lands was the only hope for healing. All of those that carried the ring, Bilbo, Frodo and Sam ended up there.

All the Ringbearers were given the option to go to the Undying Lands when the Elves left Middle Earth. Sam decided his place was in the Shire.
