nope. coconut water is the juice of the coconut (the liquid that comes out when you crack open a coconut). coconut milk is made from the meat of the coconut and water blended together and then drained.
it doesnt get inserted in the coconut shell itself but it actually produces in there if you open a coconut shell it has white stuff on the side that is coconut all the juice gets produced by that.
None if its a strawberry smoothie. Coconut is an optional ingredient. You can use as much as you like.

Coconut flesh from a mature (not green) coconut and water. These are mixed together then pressed and seived to get a liquid called coconut milk. If there is less water used then you get coconut cream. The liquid inside a coconut is very differnent. That is coconut liquor which is sweet and fairly translucent compared to milk.
You can get coconut juice almost at grocery store in Chinatown in SF but they are canned coconut juice if that is what you are finding
