I assume you are referring to the "State of the Union" speech; this is an address a president must give every year; it is a report to the congress and the the people of the United States, in which the president outlines what has been achieved and what his goals are for the next year. But I am not sure I understand your question: if you are asking whether he knows or can see who is in the room while he is speaking, the answer is yes. He knows that the room will be filled with members of congress, Supreme Court justices (if they want to be there), cabinet members, etc; and in the upper gallery, there are various family members, as well as a group of people who are being given recognition or who earned the right to attend the speech because of something special that they did. But as a professional speaker, the president is mostly paying attention to reading his speech, so he is going to look out at the audience sometimes and read his speech (which is displayed for him on a device called a teleprompter) the rest of the time.

But if you are asking about giving recognition to certain people, yes that does occur in a State of the Union Address. Often a president will mention or point to someone in the gallery, who is seated with the First Lady and other members of the president's family. It may be a service-member who displayed uncommon bravery; or a humanitarian who did amazing things for a charity; or a couple who dedicated their time to registering people to vote; or a person known for some other praise-worthy achievement. The president (whether Mr. Obama or other presidents before him) wants to highlight the actions of such people in his speech as an example of good people who are doing something positive to make the country better. To cite one other example, during President Bush's term in office, he used a moment in his address to acknowledge some Iraqis who had voted for the first time in free elections.
