For a child:

Go to the department in charge of vital statistics in your county of residence and ask for a form for an amended birth certificate. Fill it out, have your signature (or the signatures of both parents if parentage has been established or that is what you are trying to accomplish) notarized, then return it to the department of vital statistics along with other supporting documentation that may be needed.

A birth certificate may not be amended by anyone other than the person or persons who are the legal parents or guardian of a child (in other words, you cannot have your name added to a birth certificate without the legal parent's consent going through the proper legal channels including establishing parentage in a court of law).

For an adult:

Your birth certificate is your true legal name. Even if you are only trying to change the spelling, you must legally request a "new name".

Once that process is complete the court clerk can issue official change of name notices. These are what you will use to update all of your legal documents. You will need one for the DMV, for your social security card and take one to the office of vital statistics to have a new birth certificate issued with the name updated.
