It's a big job, but you can use a putty knife to get the loose paint off then 'rough sand' the rest of the ceiling and paint. Be sure the ceiling is even and if there are any dents from the paint peeling then you'll have to plaster those first, then 'rough sand.'
If the surface of the wall is drywall made from gypsum and is not wet, simply remove the peeled paint to a point that the paint does not peel easily. Primer then repaint. If it appears that the drywall has been wet and there may be significant damage to the wall simply replace the spot and primer and paint. If the damage is not to bad then you should be able to go over the affected area with joint compound sand smooth then primer and paint.

If by top layer you mean the paper, then re-install drywall on that wall.
You should carefully inspect the wall board for any damage. If the paper "skin" of the wall board has peeled off with the wall paper, you will have to patch that before you can paint. Otherwise, I would recommend a good quality latex primer.
To fix fresh paint that was peeled, you will need to remove any loose paint debris. After removing, the area will need to be sanded down. You will also need to apply primer to the area.
