The highest movie rating out there is NC-17. Which has a higher status than Rated R movies. If a movie is Rated NC-17, it means that no children under 17, no matter what the circumstances, are not allowed to watch the movie because it is stating that it is more for the adult audience.

These ratings were originally a joke between directors of films with either pornographic or artistic nudity, because they were angry that viewers could not distinguish the difference between the two types of nudity. These "pretend ratings" were eventually picked up and used to rate some films in stores today. So, the worst "official" rating currently available would be "NC-17", while the worst "overall" rating would be XXX.
