If is a hairline fracture of the a forearm bone (radius or ulna) for instance, then splinting/casting may be all that is needed - in order to allow for safe healing to occur (no pain from muscular movements in the arm, and minimal arm movements so as not to allow for further damage, while healing).And start to put presure on the minor fracture.

Answer:Remember that First Aid is all about stabilization and transport -- we aren't going to actually treat a serious fracture in the field. That said...
  1. Check patient for airway, breathing, cardiac, bleeding, and determine if the fracture is compound.
  2. If compound apply a dressing to the wound and control bleeding.
  3. Splint extremities for immobility, taking care not to move the bone ends any more than is absolutely necessary, and making sure circulation is maintained.
  4. When long bones break (and especially the femur), they often release fat into the bloodstream, which in turn can cause some disorientation and panic -- be ready for this and restrain if necessary.
  5. Treat for shock, elevate legs above the heart if it's easy, and transport.

If you're in the field and can't get transport, you have a few options. Fractures in less critical areas (arms, hands, etc.) you can walk out, depending on the patient's condition. Leg fractures and other fractures that preclude walking but are not instantly life threatening, can be littered out -- secure the patient to a litter (Stokes if you have one: fashion a travois from branches if you don't), and transport to the nearest help. Make sure you maintain the patient's body temp and hydration. Administer pain control if you have that capability. More sever fractures that require evac mean you'll need to get a call into a rescue unit (radio or cell phone if you hanve it; two runners if you don't have communications, one runner alone is at risk).

Reducing a compound fracture in the field is beyond the scope of this reply.

Notes on specific fractures.Skull Fractures: If you have cause to system a skull fracture, this is a critical injury and needs transport to an ER. While any fracture is serious, this is one of the most so. In this case, if possible, you should get rescue to come to you. Skull fractures may warrant a medi-vac.Fractured Toes. Usually the least dangerous fracture although extremely painful. You can often strap the toe to the adjacent to for support instead of splinting. If the great toe is invovled, or if pain too great, you may need to carry the patient.Fractures of the chest. The primary problems here relate to breathing. First, fractured ribs always make it hurt to breathe, so you'll need to find a poistion where the patient experiences the least discomfort breathing. This almost always means sitting up. Note also that broken ends of ribs can puncture lungs. Look for wheezing noices, bloody froth at the lips, a puffy neck (which may crackle like rice crispies if you palpate it) -- these are all signs that a lung has been peirced and that the separation between blood and air is compromised, so this is not a REAL emergency, and you'll need to transport fast. Keep a shapr eye on the patient's breathing and skin tone. If they starting breathing irregularly, or become cyanotic (pale or blue-tinged), you have trouble, and need to use utmost speed, as treatment for this in the field requires a very high level of skill. At this point the patient should be on O2 if you have it. In the field, it is possible for the patient to walk -- maybe, and you may also be able to litter them out, depending on the extent of the injury."Flail Chest", where the ribs are broken in two places, allow for a deep indentation to appear in the chest, can compress internal processes. This one calls for a medivac also. Monitor the patient for cardiac problems.Fractures of spine or neck. If you suspect this may be the case, immobilize patient, and treat for shock (preserve comfort and body temp). This one cannot be safely walked out -- you'll need to send for help to evac the patient. Once more, me extremely careful not to move the spine. If you must, get everyone to help you move the patient, keeping the head, neck and spine all in line as you roll them. Keep the patient as comfortable as possible, maintain hydration and body temp, and get help to come to you.
