A clinician is a health professional, such as a physician, psychologist, nurse, etc, who is involved in clinical practice, as opposed to one who is involved in research.

A physician is a health professional who has completed a course of education in medicine and who is licensed to practice medicine and/or surgery. A physician is a clinician, but not all clinicians are physicians.

Well a Surgeon is a physician who specializes in surgery.

A Physicain is a person who is legally qualified to practice medicine; doctorof medicine.

So the difference is the surgeon does surgery on people and a Physicain practices medicine.

Doctor is a title give to anyone with an advanced degree, which may be a dentist, a vetrinarian or may even have nothing to do with medicine. A person with an advanced degree in religious theology is a Doctor of Divinity.
