This is a cliche or saying that warns people to be careful about relying on first impressions or the way something looks, as these superficial features can hide a very different underlying reality. In this example, the appearance (good paint, good interior carpeting) deceived (lied, hid, presented false information) about the actual condition of the vehicle (engine is about to fall apart).
