When a heart is looked at without using a microscope, the term grossly is used. It was not unusual in anyway (unremarkable). Looking at the heart, everything seems fine. But looking with a microscope may show some thing else.
Usually found on the reading of a radiograph (xray, CT, etc), it means the appearance of the heart or cardiac silhouette is without abnormality.
I think you read the result incorrectly. "Grossly remarkable" doesn't make sense. "Grossly unremarkable" means there was no obvious abnormality.
