Hearts, liver, kidneys, lungs, small intestine, pancreas, corneas, heart valves. Some other things have been transplanted (both successfully and unsuccessfully), but typically only "life saving" organs are routinely transplanted.

Any except the brain, so far. Although more transplants are "possible" than are usually performed, since not all organs are vital for life. (E.g the stomach is rarely transplanted, and large intestine is never transplanted, since they are non-vital).
