There is none that stands out. Gout had been with us for all of human history and it is not until recently that we have been able to relieve ( but necessarily stop) the pain involved. This is done with anti inflammatory and analgesics (pain killers) and with these come the side effects that may at times be a concern. However pain in itself can be a thing that leaves effects that harm the body and spirit, so it is all a mater of balancing it all out.

The inflammation caused by gout needs to be brought under control first. This is often done using the drug Colchicine. Usually, joint pain lessens within about 12 to 24 hours of treatment with colchicine and disappear within 48 to 72 hours. Colchicine tablets can be taken or injections can be administered in the cases where the digestive tract has adverse reaction to it, the side effect of this drug can be diarrhoea and more serious side effectssuch as damage to bone marrow.

At present, non steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen and indomethacin are being favoured over colchicine to relieve pain and swelling in the joint.

At times steroidal anti inflamatory's (corticosteroids) such as prednisone may be prescribed for the same purpose. A corticosteroid crystal suspension can be injected into an affected joint effectively terminating the inflammation caused by urate crystals. The inflamed joint may be immobilized to reduce pain. This is done if there are only one or two joints effected.

It may be necessary for people suffering from repeated severe attacks to begin long-term drug treatment. This is begun when symptoms of the attack ore gone and is then continued between the attacks. Low doses of colchicine may be taken on a daily basis, which can either prevent attacks or reduce their frequency. Taking NSAIDs while there are no symptoms can also prevent attacks. At times, both colchicine and NSAIDs are required to keep the pain and inflammation under control. This combination does not prevent or heal ongoing joint damage that is caused by crystal deposits, and is riskier for people that have kidney or liver disease.

Lowering the uric acid level in the blood by increasing the excretion of uric acid in the urine with drugs such as probenecid or sulfinpyrazone is another strategy. Keep in mind that aspirin blocks the effect of probenecid and sulfinpyrazone and should not be used at the same time. Where pain medication is required, acethaminophen or ibuprofen can be safely administered instead. Drinking a lot of fluids - at least 3 quarts a day may also help reduce the risk of damage to the joints and kidneys when the excretion of uric acid is increased.

Allopurinol is a drug that blocks the production of uric acid in the body, this drug is particularly usefull to people that have high uric acid level as well as kidney stones or kidney damage. Allopurinol side effects can include upset the stomach, a skin rash, decrease the number of WBC, and cause liver damage.

As the amount of uric acid in the blood is decreased most tophi on the ears, hands, or feet will slowly shrink, However very large tophi may have to be surgically removed..

I got relief at night from using a blanket lifter to get the covers off my feet in bed.

However, the problem of using the blanket lifter was by lifting the blankets it created a hole in the end of the bed that made my feet cold and painful.

I found and used a blanket lifter called Somnimax that has a cover that blocks the end of the bed and insulated the tented cavity and kept my feet comfortable and warm and allowed me to get a good night's sleep.

I recommend the Somnimax to anyone who suffers from nighttime foot pain.

Staying hydrated is a good way to keep gout from returning too often. Good hydration also keeps gout inflammation from being as severe. Avoid caffeine and alcohol during a case of gout. Anti-inflammatory medications like Ibuprofen can help keep the swelling to a minimum and reduce the pain of gout after the swelling starts. Some doctors recommend taking larger doses of anti-inflammatory medicine at the beginning of the inflammation and then tapering off the medicine as the gout swelling decreases over time. Aspirin is not an effective medication for treating gout.
