In England, very little. In 1940 you were allowed to spend 1s10d (one shilling and ten pence, twenty pennies, a twelfth of a pound) per person per week on meat. B7 1942 this was down to 1s - a twentieth of a pound. Early in the war, a shilling would buy thre or four pounds of lamb, but prices rose as time went on. It was impossible by 1945 to have meat every day without cheating. Please remember also that rationing did not end with the war; in the UK, meat came off the ration only in 1954.
You can't really say. It was different at every camp. Some only received a quarter loaf of bread a day and a small cup of water. Only sometimes would they get watery soup but not often. They turned into walking skeletons.
The soldiers didn't get much. this is 1 soldiers weekly rations 1 lb oz of meat, 4oz bacon or ham, 3 pints milk, 2oz butter, 2oz margarine, 2oz fat or lard, 2oz tea, 1 egg, 2oz jam, 3oz sugar, 1oz cheese and 3oz sweets.

Generally it was transported to & fro (In Europe at least) by the Red Cross. This applies to British & US forces. The Red Cross did much to ensure the safety and survival of hundreds & thousands of prisoners. There is then a vast contrast between on one hand the treatment of German & Italian prisoners held by the Allies & the Russian prisoners of the Nazis and the German prisoners of the Russians & similarly the Japanese Allied prisoners: The conditions were appalling. There were very few Japanese prisoners: Surrender wasn't an option.
