To get a permanent unlock from a trusted vendor I use iFactoryUnlock.
They can do AT&T in the USA and a bunch of other networks world wide.
So far I have used them for 3GS, 4, and 4S unlocking on AT&T and T-mobile UK.

The machine can transfer contacts, pictures, videos, and music/sounds. There is usually a minimal fee of $10 for this service. If you got a replacement phone through Verizon wireless warranty, a corporate store will usually do it for free. If you upgraded, the store where you got the phone should have done it for free.
