Setting a code 45 means the oxygen sensor has detected a rich exhaust. There are several possible causes.
Excess fuel pressure
Leaking fuel injector
Leaking fuel pressure regulator
Intermittent throttle position sensor
Faulty MAF

On the 2.8L V6 the distributor cap is held on with two screws. Unscrew the two screws and pull straight up on the distributor cap to remove it from the distributor.
