lug bolts are pressed in to either the axle or rotor from the back. first remove the drum or rotor for easy access, using a punch push the remaining part of the bolt out of its hole, then feed the new one in, you can use a bunch of washers and the lug nut to press it in just keep stacking washers and tightening until the lug is fully set in the axle, a hammer is not recommended
