Remove the calipers on the front ones and they should slide right out. On the back after you remove the caliper , you need a screw that fits the 2 holes by the lug nuts and screw it in until the rotor pops then just slide it out. keep your parking brake off. The rear one is also the parking brake drum.
If your rotor is rusted to the wheel bearing, which it probably will be, use a hammer on it to attempt to break loose the rust and release the rotor.
when you remove the caliper on the disk brake by removing the bolts on the inside of the caliper the rotor just slides off, the only thing holding the rotor in place is the wheel.

pull off the front wheels. remove the calipers. remove the two screws that hold on the rotors. replace the rotors and reverse the procedure.
you may need to replace your rotors and brake pads. to check this remove your tires and examine the rotors/pads for excessive wear.

Step one is to remove the tire. Step two is to remove the brake caliper. Step three is to remove the cap that covers the spindle the rotor is spinning on. Step four is removing the bolt holding the rotor on the spindle. Step five is pulling the rotor off the spindle.
This change out should be fairly easy (compared to the front rotors). I changed out the rear rotor on a 2000 Sebring Convertible rather quickly. Buy the rotor at Autozone or o'Rielly and you will see that it justs slips over the lug nuts. The old one may take a little tapping around the edges to loosen it up.
