Yes it is hard and very dusty. I've had to do this a number of times and found the most effective way is to rent a large, heavy floor sander. Use 36 grit paper and sand away the junk -It gets very dusty, so have a good mask, goggles and hair covering. - oh BTW - it's pretty back-breaking too !
If your floor is hard, then you spray vinegar on it and let it sit for a minute. After that, you wipe up the vinegar and try to scrape off the adhesive.
The easiest way is with a small jack hammer or a lump hammer and a chisel
Rub it with a rag dipped in Goof Off, then try sliding a craft knife blade under it.
This is not a good idea just because of the difference in threshold height you will end up with. If you are using an adhesive backed flooring it wont stick Ceramic floors are notoriously uneven and this will cause squeaks and unsightly bulges and such.

Not directly as the ceramic tiles will soon show through the vinyl flooring. If the ceramics are laid on a wooden floor, you must take up the tiles and overboard with 6mm plywood. If the tiles are on a concrete floor, you can leave them down and lay a latex screed on top to provide you with a smooth floor for your vinyl

If your ceramic tile looks a little too dingy for your taste, it may be time to give it a hard clean. Hard cleaning is something that should be done in careful moderation and not overdone. Some of the chemicals and tools used can actually ruin your ceramic tile flooring if used too often. To give your ceramic tile a hard clean, use tools like a floor scrubbing brush, a little bit of bleach, all purpose cleaner, mild detergent and more. Once you are finished scrubbing the floor using your solution, rinse it with warm water and wipe up any residue or film with a dry cloth or towel.
If it's hard and there is a lot of it, it's easiest to rent an industrial floor sander. Home Depot rents these, as do many rental stores. Get one and save a lot of backache. -But wear a mask and goggles when you do it !
Ceramic Tile Installation is not at all as hard as it may look. Simply find the measurements of the room for which you will install the Ceramic Tile, and go out to any of your local hardware stores and buy the tiles that fit you best. All you have to do is simply peel off the sticker on the tile, and just lay it neat and flat on your floor.
