You can order a copy of a Wisconsin birth certificate from the Wisconsin Vital Records Office. You can apply in person, by mail, or by fax. Proper ID is required, such as photo driver's license or ID card, or 2 of either a government issued employee ID badge with photo, passport, checkbook, bankbook, major credit card, health insurance card, recent signed and date lease, utility bill, or traffic ticket.

You can apply in person at 1 West Wilson Street, Room 158, in Madison between 8 A.M. until 4:15 P.M., Monday through Friday. They provide regular and expedited services. You can get the application at their office or download it from the Wisconsin Vital Records website. In-person requests for certified copies are completed within 2 hours if you get there by 2 P.M. Uncertifed copies can take up to 3 months to process.

The search fee, which includes 1 copy is $20. Additional copies ordered at the same time are $3 each. Expedited in-person requests are completed in 15 to 30 minutes and cost an additional $20.

You can apply for expedited service by fax and it can be sent to you by regular mail or overnight UPS Next Day Air. You can download the fax application form from the Wisconsin Vital Records website or call (608) 266-1373 to have the form faxed to you. Fax requests will be charged for the expedited service. Fax requests to be sent by regular mail will be completed and mailed within 5 business days. Faxed requests for copies to be sent UPS overnight with be completed and shipped within 1 to 2 business days. The fee for UPS shipping is $19.

Certified copies may only be issued to the person named on the record, current spouse or domestic partner, parent, sibling, child, grandparent, a person with written authorization, or someone who can demonstrate the the record is required to protect a personal or property right. Uncertified copies are available to anyone who applies but will not be acceptable for legal purposes.

You can also request copies from the County Register of Deeds Office in the county where the birth occurred.
