From a water buffalo's horn in a stampede which he planned for his brother, Mufasa, not to kill him but to make him look bad in their father's eyes so that he would be made future king indtead of Mufasa. But Taka's plan bakcfired and he himself got almost killed in the stampede, but survived with receiving only that scar. This is all told in The Lion King: Six New Adventures Book 1: Tale of Two Brothers. The books are only semi-official, as in official in general but the film makers have denied them so they do not link to the movie's story but are a seperate entity.
Taka is a semi-official version of the character Scar, approved into a few books, but not approved by Scar's creators. Taka wasn't made by Disney but was approved by the company to the book universe only. Taka exists in a series of books called The Lion King: Six New Adventures, but there are other just as official books that contradict those books, and on the commentary track of the film, Scar's creators as in the film makers, have denied Taka by denying the Six New Adventures' books' main character Kopa. That's why it's nothing more or less but a semi-official version of Scar.
His REAL name is Taka but every lion calls him Scar because of the scar on his face. I don't know how he got the scar, though.Scar
They never mention what Scar's real name was in the film universe. In a series of books approved by the general Disney, his birth name was Taka but that is not the movie Scar's real name because it was not approved by Scar's creators as in the first film's makers - as is evident from the commentary track of the film. That name is barely semi-official, because even though Disney gave its stamp on the books, that name was made up by a man who wasn't part of Disney. As in that Disney only holds copyrights of the name, it didn't come up with it. And as said, it isn't the movie Scar's real name. His real name remains unknown.

They never mention how Scar got the name "Scar" in the film series other than possibly his parents named him that. However, in The Lion King: Six New Adventures book approved by Disney, his birth name was Taka, but that is not the movie Scar's real name because it was denied by the filmmakers on the audio commentary found on the DVD releases of The Lion King, when they denied the cub at the end of the film was Kopa, the main character of the book.
