Most airlines use the seatbelt extenders for the demonstration in the beginning when the flight attendent shows how to buckle. If, when boarding the plane, you ask the flight attendant greeting you for an extender, they will usually tell you that they will deliver the extender to you after the presentation. This is true for MOST airlines.
Also if you are a frequent traveler, you can purchase your own seatbelt extender. They are roughly 50 dollars and you can order them online.
