It depends on your age and why you're changing your name. If it's for marriage, it's easy to just Google it for the process, as there are many agencies that you need to notify in order to change your name.
If you're changing your name for a reason other than marriage, you usually have to petition the court in your local area for a legal change of name. If you're under age, then it has to be originated by one of your parents or legal guardians, and both parents must agree to the change by signing the petition. After that, a judge reviews and approves / denies the request but it's rarely denied.
In any case, the Clerk of the Circuit Court in your County is probably the best place to start, as they'll have the forms for such a process, and they'll tell you what you need to do in order to accomplish it.
Yes you can, go to the city hall and see what papers you need to fill out. * Name changes are done through the state probate court in the county where the person wishing to take the action resides. It is a fairly simple process in which the person files an affidavit with the court requesting a change of name. The person then appears in front of a judge to answer a few questions such as their purpose for taking the action. If the judge believes that the request is not being done to hinder and civil action, to commit fraud, infringe on another person's privacy or to otherwise mislead or impact the public the request will be granted.
You file the completed forms at the courthouse and attend a hearing. Some states require that you publish a notice in the local newspaper too and some states also have you sign a form or complete a fingerprint card so they can do a background check on you.

The name change process is state specific. In order to be officially recognized by government and financial institutions, the best way is to obtain a court order for a name change from your state court.
Yes, it is possible to legally change your name (in the UK from the age of 15 (I think), with the permission of parent/legal guardian).
Unless you start the whole game over, then no.
When you are an adult, usually 18, you can petition the court to change your name.
Generally, you can change your name legally through a simple court process but your name will remain the same on your birth record. Name changes do not carry over to birth records.
Yes.
If you mean can you change the birth certificate to reflect the new name when a child is adopted, the answer is generally yes depending on state law.
Putting a father's name on the birth certificate does not make him legally the father if you are not married to him. You can put his name on the birth certificate, but realize that it has no legal impact. In order to have paternity established, it has to be done with a court order.
how do I add my daughter's father name to the birth certificate? I live in New York.
Perhaps you could if he relinquishes all rights or if your new husband adopts the child. However, even if you do somehow remove the name from the certificate, the fact is that he is still the biological father. Better to move on in life; fixing this will not make a large difference to the situation in the end.
go to your local courthouse and file a petition for the change, then you will see the judge to see if they will allow the order for changing your name, this way you will have all of the legal documentation to take with you to social security office and the drivers license bureau. Added: The answer given above will change your formal identification on all official documents -EXCEPT- for the name on your birth certificate. That is an 'unchangeable' document and the information contained on it will always remain.
You can't change your original birth certificate but you can change your name. See a lawyer or get married.
yes you can

In some states if a person requests a change to their birth certificate such as a change of gender or change of name the actual birth certificate is updated as if it was always that way. In other states an affiliate of correction is issued. It usually consist of a copy of the original birth certificate with a certified statement showing the correction and stating that this is a valid birth certificate for the stated person.
You may NOT change the information contained on your birth certificate. It is an official record of your birth, the date it occurred, where it occurred, your sex, your birth name, and who your parent(s) of record were.

You need to have the cooperation of all parties in order to effect a name change.

No, that is simply silly
The state does not take back your original Certificate. You will now have 2 documents one showing your name at birth and the new one reflecting the change. No one else will ever be able to access the original certificate. Only the new one documenting the change in your name will be available. But it does not make the first one disappear just because you have another one reflecting the change.
