I believe that yes, a first cousin can make medical decisions in the absence of estranged blood siblings because if it's an emergency and that's the only family member there in that moment someone will need to make a decision and the cousin would be the best person for that. At least, this is how I see it. If my life was in danger, meaning that I was the person in the hospital, and the only family member there with me at that moment was my fist cousin I would prefer for my cousin to make a decision than to die without even trying.
