Not always. Some people have just been so hurt in their past that they seek approval of their peers and just want some TLC. It's best to get to know the person a little before making a judgment call. It's a good policy not to label people immediately or even after a few weeks because you too could be labelled.

No, typically pathological liars are not not aware of the fact that they are lying constantly. And, often times they believe their own lies. However, we can't mix pathological liars with diagnosed disorder with a person who lies to protect himself/herself.
