Typically, there are two accepted paths of reason for why a cat (male or female, neutered or not) would exhibit this kind of behaviour if it had not been doing so previously.
Psychological: Your cat may be trying to tell you something. Could you cat be distressed about something? (like something/someone familiar being absent or man-handling by children) Is the litter too smelly/dirty or being used along with another cat that is bullying or territorial?
Health: Is your cat older? Perhaps it's displaying symptoms of an alzheimer's-like cat disease, or maybe it is sick and having near-litter accidents.
Suggestions for discouraging this behaviour include keeping the litter very clean; clean it out daily. Maybe try a new litter brand. Keep watch on the cat for a few days for any signs of physical or emotional discomfort and provide companionship and reassuring patting (provided the cat wants companionship - don't smother them). if you have more than one cat, consider getting a second litter box - this may be a turf war.

It might be a health issue. If the cat has pain when going it might associate the litter box with the pain. Best to take it to the vet to see if there are health problems. If you have changed litter brands or types, that could also be the issue.
